Price effect
Add, deduct, no-charge, allowance reconciliation, or time-and-material authorization.
Schedule effect
No change, added days, pending lead time, or to be determined after approval.
Documents attached
Photos, sketches, product data, revised plan sheet, invoice, estimate, or email thread.
Approval
Owner and contractor signature, date, and approval method.
Short approval language
By approving this change order, the owner authorizes the described change and accepts the stated price and schedule effect. Work outside the written scope remains excluded unless separately approved.
